So how *do* these publishing companies set about the task of converting
from XML to PDF? Do they do it by hand???

Not sure of the publishing industry, but I know that in the printing industry such tools as StreamServe and Doc1 are used, that indeed have some proprietary way of converting XML into any output format, amongst which PDF, MS Word, TIFF, HTML or whatever format you can think of.
