If you want to output the name of an element you need touse name()
your stylesheet uses text() which extracts the text node child of the
current element.

<xsl:template match="/project/namespace/querySubject/queryItem">

That match matches elements in no namespace, but your elements are 
in the namespace
xmlns="http://www.developer.cognos.com/schemas/bmt/32/3"

so you need
to add
xmlns:c="http://www.developer.cognos.com/schemas/bmt/32/3"
then use

<xsl:template match="/c:project/c:namespace/c:querySubject/c:queryItem">

and prefix other names in Xpath expressions that refer to this
namespace.
