RE: preceding-sibling:: while keeping xml well formed

Try something like:

<xsl:template match="b//i | i//b">
  <bolditalic>
    <xsl:apply-templates/>
  </bolditalic>
</xsl:template>

In your example the b is an ancestor or descendant of the i, not a sibling.
