xsl transform flatfile to xml

hi,
I have an xml file (from an sql query)

          <o>o1</o>

          <c>c1</c>

          <l>l3</l>

    </line>

    </line>
</output>
 
which i want to transform into an xml in the following format.

The xsl i used was

but i dont get the <o> elements properly.
Any help!!!!!!!!!!!!!!
