The usual reason for setting a default namespace in an xslt file is to
set the default namespace on literal result elements in the _result_
typically the result namespace is different from the default namespace
in the source, so having xmlns=.... on an xsl:stylesheet change the
meaning of QNames in Xpaths would be very disruptive.
