Neither XSL1 nor XSL2 draft have a command called xsl:document.
perhaps you are using an implementation of the old XSLT 1.1 drfat (which
was explictly killed off and never taken past working draft status)

Why is it not?
If it is declared in your first stylesheet then it will appear in the
second if you have any literal result elements at all. perhaps you are
generating everything with xsl:element?
If that is so you'll need to copy the namespace nodes explictly
from your 1st stylesheet to your generated one
<xsl:element name="xsl:stylesheet">
<xsl:copy-of seelct="document('')/*/namespace::*"/>
