change <xsl:stylesheet xmlns:xhtml=http://www.w3.org/1999/xhtml"
to
<xsl:stylesheet xmlns=http://www.w3.org/1999/xhtml"
which makes
<html xmlns:xhtml="http://www.w3.org/1999/xhtml">
redundant (inside your templates, not an imported or source document)
