Just add another template rule:
<xsl:template match="/*">
<xsl:copy>
<xsl:apply-templates select="@*"/>
<new-element/>
<xsl:apply-templates select="node()"/>
</xsl:copy>
</xsl:template>
Note that the attributes must be output before any child elements.
