You are presumably using the html output method rather than the xml
output method. In SGML (and so HTML) PIs end with > not ?>

You said you were generating xhtml, if your top level html element is in
the xhtml namespace your system should not have defaulted to html output
but perhaps you generated an html element in no namespace?
