xpath axis preceding
ancester and descendent are rather expensive: essentially between the two
of them you are always searching the whole document.  You want
parent:: and child:: here (or more simply ../ and nothing as child:: is
the default axis)

this is a grouping problem and as usual can be optimised using keys,
(see jeni's site) but get it working before trying to optimise.
