Note that the spec requires fo:inline as the first child of fo:footnote, so the most correct markup is:

     <fo:footnote>
       <fo:footnote-body>
	 <fo:inline/>
         <fo:external-graphic src="url(foo.jpg)"/>
       </fo:footnote-body>
     </fo:footnote>
