I think that the problem is simply that you're missing a t prefix on
the xsl:with-param element here:

>            <t:call-template name="markup_attributes">
>              <with-param name="input"
>                select="regex-group(3)"/>
>            </t:call-template>

Try:

  <t:call-template name="markup_attributes">
    <t:with-param name="input" select="regex-group(3)"/>
  </t:call-template>
