  It was indeed (a). Thanks a million Dan and Micheal. I had commented out 
	 <xsl:output method="html" indent="no" encoding="UTF-16"/>


But if the html you are generating is valid then its top level element
will be <html and in that case the html output method is the default and
xsl:output method="html" is not needed.
