 This approach I borrowed from one of Jeni Tennison's XSLT utilities.
<xsl:template match="math" xmlns:m=http://www.w3.org/1998/Math/MathML>
<xsl:variable name="name"><xsl:value-of
select="concat('m:',name(.))"/></xsl:variable>
<xsl:element name="{$name}">
<xsl:for-each select="@*">
<xsl:attribute name="{name(.)}">
<xsl:value-of select="."/>
</xsl:attribute>
</xsl:for-each>
<xsl:apply-templates mode="m"/>
</xsl:element>
</xsl:template>
This should accomplish exactly what you want. Giving every attribute and
element in <math> inclusive an m namespace prefix.
