did u try giving the name of the php in the Form element..
like
<form name = "myform" action = "http://path.to.url/query.php?insert=Hello">
<input type="text" insert="{@insert}"/>
<input type = "submit"/>
</form>
add this code in the template you are defining..please let me know if this is not what you want...
