this question was asked by someone else (and answered) earlier today.

The namespace is there because you used xsl:copy on the doc element and
that copies all namespace nodes of the current node.

So either you need to be more careful with your copying or, simpler,
copy everything but then remove the unwanted doc namespace using 
the exclude-result-prefixes attribute.
