The default built-in templates cause every node in the tree to be visited.
If nodes aren't being visited, it is because you have written rules that
override this behavior. In the example stylesheet you have given us, all the
nodes are being visited. Perhaps what's wrong is that you don't notice that
the nodes are visited: for example, if an element is empty, then there will
be no evidence of the visit, because the default template rule will output
nothing.

Where does the generated ID come from? The results of generate-id() are not
predictable in advance, and can change from one run to the next.

I think you have edited your stylesheet before reproducing it below: see the
misplaced xsl:transform end tag. If you show us the actual stylesheet, your
error will probably become clear.
