undefined prefix in xsl

This seems a ratherclear error: you have used the xsi: prefix in your
stylesheet without declaring a namespace for this prefix 9it's normally
best to put the xmlns:xsi="..." in the xsl:stylesheet start tag, so that
the prefix wil be in scope fro the whole stylesheet.
