Please don't call elements by the name "entity" it makes your
descriptions very confusing for anyone trying to help. In XML "entity"
has a well defined meaning, and it means something different to
"element" which is what you are creating here.
To answer your qustion, at the top level of your stylesheet go
<xsl:variable name="x" select="."/>
<xsl:for-each
select="document('start.xml')/START/*">
......
then at any point you can get back to the original node by
eg <xsl:apply-templates select="$x/some/x/path/from/the/original/node"/>
