something like
<xsl:key name="MyKey1" match="b/foo"
use="concat(generate-id(..),':',*/x,':',*/y)"/>
or if b isn't always a direct parent, something like
<xsl:key name="MyKey1" match="foo"
use="concat(generate-id(ancestor::b[1]),':',*/x,':',*/y)"/>
then
<xsl:template match="b">
key('MyKey1',concat(generate-id(),':','1:1'))">
will give you all the foo children of b which have x and y grandchildren
with values 1 and 1.
