Hmm... interesting.  Wouldn't it be enough, then, to have:

  <xsl:output
     method="xml"
     doctype-public="-//W3C//DTD MathML 2.0//EN" />

and say that processors should use the doctype-public value to
determine which entities should be escaped and so on?
