You are using the wrong namespace:
<xsl:stylesheet xmlns:xsl="http://www.w3.org/TR/WD-xsl">

Use this instead:
<xsl:stylesheet version="1.0"
xmlns:xsl="http://www.w3.org/1999/XSL/Transform">
