As much as I hate some of the long sources posted here, perhaps we need a little more context to see where the problem lies. Could be you're using an overly forgiving transformer that's second-guessing your errors and getting it wrong. However that would make the transformer very very non-compliant, XML is NOT as friendly as HTML.
