xsl extension element prefix

Now I have tried with both XT and Saxon, and with this definition

<xsl:stylesheet xmlns:xsl="http://www.w3.org/1999/XSL/Transform"
		version="1.0"
		extension-element-prefix="doc"
		exclude-result-prefixes="doc"
		xmlns:doc="buh"

I still get the <doc:test> tag echoed to output, along with its content. What am I doing wrong here?
