I changed the namespace declaration back to
<xsl:stylesheet version="1.0"
xmlns:xsl="http://www.w3.org/1999/XSL/Transform"> 

is there one that I should be using that is newer than this?

The command line that I am using in Saxon is "saxon -o index.html index.xml
index.xsl"
That's what it tells me to use on the Saxon website.
