Jeni suggested this in a later response - it's a much more
satisfactory (XSL ethical?) solution.

You don't need to use a fallback. This is actually what the
exclude-result-prefixes attribute is for - previously I'd
only used it for removing declarations of namespaces that
I'd use when matching elements in my input documents.
