fo:display-graphic usage with FOP

I have been using FOP to successfully create a PDF file by inputting my XML and
XSL.  I tried to add an image using the fo:display-graphic command and
href="filename".  The PDF file is created successfully, but when I try to view
it the Adobe Reader image appears and nothing else is viewable.  The PDF reader
says there are 3 pages, but they are all grayed out.  What am I doing wrong?
