You can try to replace <IMG SRC="{@PlanLogo}" BORDER="0"/> with:

<xsl:element name="IMG">
  <xsl:attribute name="SRC">
    <xsl:value-of select="@PlanLogo"/>
  </xsl:attribute>
  <xsl:attribute name="BORDER">0</xsl:attribute>
</xsl:element>

That may work...
