Usual way is:

<xsl:stylesheet 
  xmlns:xsl="http://www.w3.org/XSL/Transform/1.0"
  >

You don't have to use the xsl: prefix, but most people do, but the
namespace URI has to be _exactly_ that, with (eg) capital T.
