fo page sequence format
The initial-page-number is necessary if I have multiple sequences. I may
wish to either start the numbering anew, or continue with the
next-available number following the last number used in the prior sequence
(I must assume I don't know the length of the previous sequence). Both
initial-page-number and the format capabilities are required.
