None of these should be the way to do it, but:

 - Use vendor extensions

 - Do one or more pre-passes to get the size of things
   from the area tree

 - Run the XSL-FO formatter from within your XSLT to
   iteratively size things to fit

 - Wait a couple of years for CSS to overtake down
   a different path

 - Use TeX (some would say that is *the* way to do it)
