The biggest problem is that the spec tries to make prefixes non-significant and fails. Another problem is that the spec is vague as to what constitutes a valid namespace URI (and whether it is ever meaningful to dereference a namespace URI, resolve it against another URI, etc) The root cause of the problem is that the layering is wrong: naming is fundamental to XML, and you can't add something fundamental as an optional layer on top of the core.
