There's a possible under-the-counter exception. Both xml:lang and xml:space are mentioned in XML 1.0. At least in the original version there is no mention of "xml" in that context being a namespace prefix but isn't that what it is?

That, implicitly refers forward to the statement, "The prefix xml is by definition bound to the namespace name http://www.w3.org/XML/1998/namespace." in the Namespaces in XML Recommendation.

In other words, de facto, XML 1.0 uses a namespace URI and prefix not explicitly declared at the time that the first edition of XML 1.0 was finalised.
