The DOM WG is working to define a new "normalization" method that will fixup things so that a document that has had new elements inserted in a namespace gets the appropriate declarations added.  Details still uncertain.  If you're a W3C member, by all means look in the DOM archives and offer practical advice.  If you're not, please look closely at the next public working draft. Since the Xerces people are well represented on the DOM, it's a safe bet that the DOM will incorporate what they've learned from their attempts at fixing up the declarations into the spec.
