errata for XML Elements of Style

_XML Elements of Style_ has been out for about six months now, and I've
slowly (fortunately) compiled some errata for it:

If anyone has more errata, I'd love to collect it (and I'll credit it on
the errata page), though I'd prefer to hear about it off-list rather than
on xml-dev.

I'm also in the early planning stages of a revision of XML: A Primer, so
I'd be happy to hear about errata for that book as well.  (We'll be
attacking typos anyway, and creating new ones, so please stick to factual
glitches.)

This is kind of a scary request to post.  Oh well.
