Andrew Layman wrote an excellent article on the Microsoft site about
converting between trees and tables but I could not find it when I tried
to look.  Perhaps it was so good that no-one else has needed to treat
this issue!

Does anyone have a good reference for this. (It was co-authored, and
perhaps written in 1998.)
