The definition in section 4.4 of "Reference in Attribute Value" explicitly includes default attribute values, and the entry in the table for "Parameter" and "Reference in Attribute Value" is "Not recognized". To re-iterate the point: in the example above, "%e;" is not a reference to a parameter entity, it is just the characters %, e, and ;. This is consistent with SGML, which allows "replaceable parameter data" in "parameter literals" in "entity text", but not for "attribute value specifications".
