I missed any ansers to these, but would hope the answer to (2) is "no".
To do it this way would conflate the identity of names with the rules for structure.
This would be a bad idea.
For example, it would permit well formed constructs in which that it would be
impossible to determine the element declaration until its attributes have been
parsed. Is this really necessary?
