In your first example, that DSSSL book author does not use that example as DSSSL style-sheet. What he want to say in NOTAION is "This is DSSSL file". I think, about our idea, style-sheet entity's notation should mean "THIS IS STYLE-SHEET which described in DSSSL". This idea needs STYLE-SHEET notation.
