I get this error when trying to create an XSL:FO template, by entering <fo:...> ... </fo:...>. What should I do?

Shaul,
You need to map the fo: namespace in the stylesheet element, something like this:

<xsl:stylesheet version="1.0"
xmlns:xsl="http://www.w3.org/1999/XSL/Transform"
xmlns:fo="http://www.w3.org/1999/XSL/Format">
