It's not clear if you need to write an XSLT that generates the HTML that displays the form, or if you need the Javascript inside the HTML form that builds the XML fragment following that schema.

Hi Dominika,
if you need the XSLT to generate the form, you can directly type the HTML in the XSLT stylesheet, as it doesn't seem to be dynamic (i.e. driven by the XML described by the schema fragment). If you are not familiar with the HTML syntax to build the form, you can use an HTML editor to design it, then take the generated code.
