After I customize the debug toolbar, the Reset functionality does not restore the moved or removed buttons.

That does look like a problem; you may want to "manually" reset the toolbar state deleting this registry key (if you are running 2006 Release 3; the entry is similar for Release 2):

HKEY_CURRENT_USER\Software\Stylus Studio\XML Enterprise Edition 2006 Release 3\DockingWindows82-Summary
